Welnote is a healthcare coordination and asynchronous case review platform designed to support healthcare programs operating in low-resource, remote, and humanitarian settings.
The platform helps field workers, healthcare professionals, and program supervisors coordinate patient follow-up, document observations, review cases, and track care activities over time.
Welnote is designed to support continuity of care and healthcare program operations. It is not intended to replace direct clinical care, local medical services, or professional judgment.
Intended Use
Welnote may be used to support:
- maternal health programs;
- child health programs;
- chronic disease follow-up;
- preventive and community health programs;
- longitudinal patient monitoring;
- healthcare program coordination;
- asynchronous clinical review; and
- documentation of care activities and follow-up plans.
Healthcare professionals may review patient information and provide recommendations through the platform. Final clinical decisions remain the responsibility of qualified healthcare professionals and participating healthcare organizations.
Not Intended For
Welnote is not designed, intended, or validated for:
- medical emergencies;
- emergency triage;
- ambulance dispatch;
- trauma management;
- stroke management;
- heart attack management;
- surgical decision-making;
- intensive care management;
- life-threatening conditions requiring immediate intervention;
- real-time emergency consultation; or
- situations where delays in communication could create significant patient risk.
If a patient requires urgent or emergency care, users should immediately follow local emergency procedures and seek appropriate medical services.
Platform Role
Welnote provides software tools for healthcare coordination and documentation.
Blue Commons Inc. does not provide medical care, diagnosis, treatment, nursing services, emergency services, or telemedicine services through the platform.
The platform does not make clinical decisions and does not guarantee clinical outcomes.
User Responsibilities
Healthcare Professionals
Healthcare professionals remain responsible for:
- exercising independent clinical judgment;
- reviewing information appropriately;
- considering local circumstances and available resources; and
- determining appropriate recommendations and follow-up actions.
Field Workers
Field workers remain responsible for:
- accurately collecting information;
- documenting observations in good faith;
- following local program procedures; and
- escalating urgent situations through appropriate local channels.
Program Operators
Healthcare organizations and program operators remain responsible for:
- patient care delivery;
- staff supervision;
- patient consent processes;
- local regulatory compliance; and
- clinical governance within their programs.
Connectivity and Operational Limitations
Welnote is designed to operate in environments where internet connectivity may be limited or intermittent.
Users should be aware that:
- synchronization delays may occur;
- information may not be immediately available to other users;
- device failures may affect access to information; and
- communication through the platform should not be relied upon for time-critical medical decisions.
Safety Principle
When there is uncertainty regarding patient safety, urgent clinical needs, or the severity of a patient's condition, users should prioritize direct clinical assessment and appropriate local medical services rather than relying solely on information available through Welnote.
